I am not a psychologist, though I have a BA in it. A diagnosis by a qualified clinical psychologist is needed for an official diagnosis of NPD. That doesn't mean that someone without an official diagnosis doesn't have NPD. They can obviously have NPD without an official diagnosis. I've rarely encountered someone who exemplifies the criteria for an NPD diagnosis more than president Trump. If Jeb Bush were president, he would not be an example of someone who exemplifies NPD, as far as I can tell, he's not suffering from any personality disorder but may be depressed (understandably). GWB doesn't have a personality disorder, but may be suffering early signs of dementia. Bill Clinton clearly has issues (possibly to a criminal degree) regarding the way he relates to women, and may finally be starting to show signs of mental deterioration due to age himself, but I don't know of a mental health diagnosis that really fits him.
