As it happens I have a replica black-powder revolver and it is a single-action which means that the hammer must be cocked with the thumb which advances the cylinder to the next round and then the trigger releases the hammer. So, in order for what was described to happen someone took a lot of time to load the gun and the 12 year old must have deliberately cocked it.
Sounds fishy.
The suspicious series of events is common in nearly all "accidents". Many modern firearms have more safeties than the old ones.
