When my eldest was almost 4, he still used it to sleep.... and was pretty much like you describe.
One day, he was having a tantrum over something, my wife got fed up, took it out of his mouth and threw it behind the furniture... There was no other to replace it, so he had to go to sleep without it.
It was a sort of punishment for the tantrum, but he did go to bed and slept quite well.
A few days later, he asked quietly for the pacifier, but we told him we didn't have any, and he was now a big boy and didn't need it. He took it like a man and went to sleep.... and never again did I have to use those devilish devices.
My other two kids never liked them.... luckily, they never resorted to sucking thumbs either!
