As in, what would cause us to extinction or what would cause us the most suffering? I'll take the second one.

I'm taking low probability threats like meteor strikes and the third world war out of it, although they would cause great suffering.

1. Global warming
2. Dietary diseases
3. Tyrants and theocracies

All of these threes are actually going on right now, dayum.

For me it has to be militant islam first, aggressive virus next, freak meteorite strike, self suffocation of the planet...

In no particular order.

Asteroid/comet impact

Nuclear war(Most people have no idea how close they have been to dying via nuclear bomb).

Disruption of our current orbit.

As long a nuclear weapons exist in mass numbers like they do today, there is a potential for them to be used in a way that destroys humanity. If that condition exists for enough time potentially will give way to actuality.
