I can only assume you're talking about Montgomery Clift and The Clash's "The Right Profile."
To be fair, the car crash injuries weren't exactly disfiguring (at least not after the plastic surgery), but it did cause him to age a decade or two between shots in Raintree County.
He certainly didn't need to be shot from a certain angle, as this video with shots from The Misfits (shot four years after the accident) show:
