        I do understand how SRS works, I knew a person who went through it and it was very messy. I still don't see how something as breast implants can be covered under the guides of good therapy. I can see breast reduction being covered since cases have proved that it is medically harmful, I'm struggling to see why getting enhancement would be deemed medically necessary.

It seems like you are clearly seeing depression not as a disease but as some sort of failing. If a person has been in therapy for years because of self esteem, has real life consequences from their depression, and a doctor medically recommends an augmentation as a course of treatment (this would only be in extreme cases, as most people would likely have success in therapy) then that should be covered if your plan already covers prescribed elective procedures.

A prescribed elective procedure would include an ACL reconstruction or a SLAP repair (both elective procedures that I've had), as there are courses of treatment that do not require surgery. If your plan covers those procedures, it should also cover any other prescribed elective procedure.

Other reasons for augmentation are uneven breast size, reconstruction after loss of size after pregnancy or cancer, mastectomy reconstructions, etc.
