Massive electronic failure and/or catastrophic mechanical errors are frequent causes of aircraft casualties. Far more often than crew malefesance. Remember that airliner that blew up off New York a few decades ago? Lots of people saying it was a stray Navy missile. Turned out to be a bare wire in a fuel tank. If the plane had been in the air for fifteen more minutes it would have disappeared, plain and simple.
Not sure about that, my understanding is some degree of human error is responsible for over half of all fatal crashes.
