Well, when people get very sick and have like a terminal cancer, they start losing weight fast, so much so that they pretty much die of hunger because they can't eat anymore.
So it is kind of understandable that people would ask you that when they saw how you were suddenly losing weight. Sickness, let alone dying, are something that people rarely talk about, so many people are in a bit of the dark about it.
