Take the undesired hormone, remove. Replace with desired hormone. Or in non-trans cases, do other such things.

In some cases, and this is just my theory it may be completely wrong, i think poor people do this in far east asian places for example, in order to be able to make money easier. Being a woman its easier to make money as a prostitute, but then they would have had to have quite a bit of money in the first place to be able to afford the op so im not sure if im right on this one
To simply make a vagina isn't particularly challenging, and it isn't where the large cost comes in. To make a pretty vagina, which might be able to deliver someone an orgasm, which won't be chronic pain for years to come, and with a high survival rate under anesthesia and with proper medical care afterward?
