Well if it's being wrong on a question you couldn't really figure out, then it's no fault of your own. For example lets say a teacher gives a math test and there is one question that says "how many cylinders does a 1997-2004 buick regal have?" Well not everyone is going to know that, and being wrong is fine in that case.
