Honestly... safe and sane are irrelevant when regarding 'morally acceptable' (from even a moderate perspective).

Instant ramen is good and all, but adding some spice really gives it flavor. It isn't that vanilla can't be (or isn't) fun... it's that it can become somewhat boring to those who are used to a whole lot of spice.
Hell, I would note that 'off the wall sex' is very much the spirit of BDSM... they don't just want sex: they want SEX. The very thought of 'vanilla' is generally more aimed at missionary and doggystyle than it is 'sex using bodies only where all pain is completely unintentional, complete with zero roleplay, zero sensory depravation, and both all parties are considered completely equal in all facets of the lovemaking'.
People just don't tend towards thinking like that
