Well I'm a Rad Tech, and people refuse CT scans just about everyday. Most of the time they are afraid of the amount of radiation they are going to receive, which is fucking stupid IMO. So yes, people can refuse treatment. People leave AMA (Against medical advice) frequently as well. No idea about the vasectomy. My guess would be that they'd be able to have the procedure.
Whether or not their insurance will cover it is a different story.
Beliefs are what divide people, doubt unties them.
