I guess they had the mythological origins of the word in mind. For example in German the word is Alptraum, which literally means 'elf dream', from the germanic mythological elves which were supposed to sit on the chests of sleeping people and cause bad dreams. (Unless I'm getting my mythologies mixed up?)
It's similar in my native tongue, though the word isn't used anymore.

Evil spirits? Bosh. Everyone knows (or should do) that nightmares are invisible, insubstantial black horses - always female, natch - who reside in the Gourd Realm, where they are presided over by the Night Stallion. The nightmares solemn duty is to bring bad dreams to the sleepers of Xanth. This is in no wise evil, as bad dreams are frequently instructive and useful to the sleepers. Here endeth the lesson.
