I wouldn't put forward any one thing and say that's the issue, the few things I mentioned are common issues, being nervous about diversity is very much not a common issue as far as I can tell with school shooters.
If the shooter is spoiled and raised to be selfish that's a family issue, the shooters are normally about as middle class and white as the victims they're shooting.

Don't put padlocks or pile a bunch of shit in front of the emergency exits. WE DID IT!! WE SOLVED THE GREAT EMERGENCY EXIT RIDDLE OF 1981!!!!
Addressing the other very relevant points is much trickier though. Not putting schools in skyscrapers is a start. Use a normal emergency exit, rather than a complicated rube goldberg emergency exit in schools probably allays some of your concerns. Finally, I'm thinking some sort of drill that practices what the kids/teachers should due in the event of a fire. Where they all go out these forbidden exits of doom, so they will find it's not so scary after all. Maybe we could call these 'fire drills.'

Honestly - for him to have to sit in jail and be forced to think about what he did, and have to live every day like that, is a good thing. Why should he get the death penalty, only to have his life end, and NOT be forced to think about the atrocities he's committed?
They should do more and confine him to solitary so that he has to sit in a 5x8 cell for 23 hours a day, never seeing the light of day again.
