No I did not miss that part.

It still means someone needs better protocol to avoid that in the future at a minimum.  This is no different to me a long term thing that changes airport safety over decades. 

Accidents like this, or plane crashes, and trucks, are mainly due to not following protocol, and or being too much of a rush. 

Business CEOs can push boat and airline and truck drivers into situations like this.

Even pizza delivery drivers have pressure put on them to drive in questionable conditions.

I don't see an excuse for this knowing weather forecasts and weather radar is widely available. So if you are not going to blame the Captain, then the company should be blamed for setting up the protocol that allowed the captain to take this risk.
I cannot find the video. But several years ago I saw video of a multi million dollar Yacht being put in the water, but they didn't take into account the weight of the boat, and the straps broke, and the boat flipped over upside down in the water. Fortunately nobody was aboard. It was a custom boat just built, never used, brand new.

OOOPS.
