Define "severe".
Severe enough that surgery is needed to deal with it. Though I should be clear I'm not against the military-funded nonsurgical treatments for body dysphoria. But surgery isn't always necessary or the best treatment for body dysphoria with Gender dysphoria it's often the best bet to resolve the issue. Of course, mixed with peer support, etc

Yes of some sort. But that doesn't always mean surgery. Other body-affirming therapies or even some chemical therapies can often resolve the issue. With Trans people, those are far less effective and the transition is the best bet.
