It's a bit inaccurate to say Hell is real. There are actually many, many Hells. I have been to several myself, and that's how I know that in order to get there from where we are right now, one must first leave the Nonfictional Plane and venture into a story world with one or more Hells in it. At that point it's mostly a matter of dying, using a portal, casting a spell, having a spell go horribly wrong, crossing paths with a powerful demon...the list goes on.
