It takes a LOT for a guy to snap. Even with someone like me with anger issues it takes a LOT to truly snap and go full on crazy mode. If he snapped for totally no reason-just like that - then he was probably mentally ill,HIGH level crazy .  
I still don't understand why you or the hospital officials didn't report it.

Depends greatly on the individual.

I've seen people snap over the most ridiculously small reasons.

Hell, I'm a pretty stable individual (no, REALLY, I am!!) and I've snapped for trivial reasons.

Playing Cluedo with my mum while I was at Uni:

"You did WHAT?  With WHO?  WHERE???"
