American Airlines used a procedure to remove and replace the engine contrary to the procedure mandated by McDonnell Douglas, manufacturer of the DC-10. That procedure led to the shearing off of the left engine during takeoff. The DC-10 design should have included failure modes to tolerate this so there is blame to spread around,
