In 2008 few Americans (including Democrats) were willing to accept the necessity of universal healthcare so I doubt Obama could have passed anything he wanted. People needed a taste in the form of Obamacare. The problem in the USA has been the majority of people who fear universal healthcare. It's an uninformed electorate that is at the heart of your problems with getting affordable healthcare for all, not the politicians.
I think (hope?) that if millions are denied insurance by the republicans there will finally be a wake up call to say enough is enough even from the red states. The free market leaves too many uncovered, does not produce better results (as compared to other nations) and isn't even cost effective compared to literally every other nation on earth as it's by far the most expensive system.
And let's not even discuss the bankruptcies that lack of health insurance causes, or how employer based coverage keeps the workforce immobile when health issues occur and keeps American employers at a competitive disadvantage because of the high costs.
The clownservatives are fools who would rather remain ideologically pure than do what works.
