Believe it or not, I know a guy who had a hang fire and turned the gun around to look down the barrel, on two different occasions. He didn't need to have the barrel already pointed at him. The Range Officer (not me, those times) kicked his ass off the range permanently after the 2nd iteration. He reloaded his own ammo, Rolleyes  so who knows what he did there; maybe it only had a primer and no powder.
