Most people don't. For the simple reason that most of big pharma's products were made through government funding of educational or health institutions or made directly by government body or department.

Most of what big pharma puts towards R&D is advertising and shilling to doctors (and in the US patients). They get their products by buying them in, mostly at knock down prices.
