I still can't pee at a urinal without this symptom and that's how I know that, in my case, it's psychological. Social anxiety in that case. Also my penis size is fine I just don't find something particularly comfortable about urinating in front of total strangers. I'd rather piss in front of my girlfriend (or on her, but that's another topic).
Moving away from my abusive ex-roommate and living alone where I'm infinitely happier magically made me able to pee normal, who woulda thunk it?
So there can be many causes, but in my case it was stress related.
