Your questions are flawed. #6 has no option simply for fearing death out of loving life.
I fear death because I love life, and I love living, I do not want it to end. All the options are about death.. not life.
Question 7 listed atheism and agnosticism as faiths, but fine whatever.. but you then contradict with Question 8. Also, I am both Atheist and Humanist..... so how do I choose a single option there.
How is it even possible for an atheist to choose on question 8. I have no faith.. but how strongly do I feel about atheism, probably about the same as Dawkins, a 6.9 out of 7 for largely the same reasons.
So do I have no faith, or rate my atheism as a near certainty... its confusing, you are abusing the word faith badly.
I've done it because I'm nice like that, but your study has many fundamental flaws, especially question 7.
