I actually, seriously know a fair bit about apples, but what you've given isn't enough to go on.
You best bet is to take some samples (an apple or two, a few leaves, and some of the soil under the drip line of the tree) to your local ag agent (that's what we call them here. I'm sure you have something similar - local offices of what ever government agency deals with agriculture). They can likely tell you what type of apple tree it is, why it's suffering, what it needs, if there's a soil issue, etc.
But don't get your hopes up. You might have a type of tree whose apples are supposed to be 'small and sad looking' - crab apples or something similar.
