In the sense that I misread a situation, yes. I say what I think. If what I think was based on a misinterpretation of the facts or if I allowed some unverified facts or hearsay to color my posts, what I think and post would be wrong. My lack of perfection completely pisses me off, while I readily admit to this condition.
I used to design and build things, and write work instructions for both the manufacture and assembly of those things. An error on a drawing and a subsequent failure of ability to assemble said thing, or an instruction that is incorrect, I take very personally, in spite of the fact that the design review process involves many people from multiple disciplines, including the people who will manufacture the pieces and the ones who will assemble them. While I realize that this chain of events has a large cast of characters, I still feel personally responsible when "tab A doesn't fit into slot B, or has to bend up, not down". I've been retired now just a bit over 4 years and am finally sleeping pretty much through the night without waking up in a cold sweat that I've hosed a design that cost many millions to fix (OK, my worst mistake only cost 1/4 Million to fix) when it didn't work exactly as designed. Of course, changes and/or repairs/rework are always required when a new thing is being made for the first time. But I hold myself to a (probably too) high standard of performance. When I worked as a mechanic, I NEVER had a customer come back with a brake failure after I worked on their car. I'd've had a tough time dealing with a death or injury due to my making a mistake. I couldn't tell you how many times I've finished a repair job and went back over it again before the customer picked it up.

Fireball, I need to tell you this.
When I was a netAdmin, (looking after all out NT servers over 3 sites), I kicked off a regular "defrag" on one of our Exchange servers (mail), (eseutil for those old IT buggers here). Anyway, these things run over night.
I accidentally closed it off the next morning and corrupted 3 months worth of corporate emails for our main site (head office). Anyway, that company liquidated years later (economic reasons -Textiles!). 10 years after they liquidated the guilt and the sweat I feel around my neck is very real. It happened to me again driving home from work yesterday thinking about it.. So I hear ya loud and clear.
