All these things can lead to double, triple, or quadruple posts. Put simply, it's a browser / user issue, not a forum one.

I don't double click it or click the button multiple times (or at least not intentionally, or knowingly :S).

I really have never seen more than two posts at once though... 4 was kind of startling :S
