A gel with lidocaine will help, I used it after gallbladder surgery.  If you have to go barefoot, tape a tongue depressor to your foot so it's about an inch ahead of the toe. This will deflect "jams".

Why not iceream sticks instead of tongue suppressors? I'm speaking in economical terms.
