It's fucking simple. If people have 10 kids in the hope that at least some survive, then the population grows.
If people have just two in the knowledge that vaccines ensure both survive, then they will do that instead. And the population shrinks.
A big part of that is the empowerment of WOMEN over their own reproductive lives. But you are dead set against that for religious reasons.
