I'm desensitized to horror.
Growing up, there were commercials advocating for the scariness of Stephen King horror, and I was reading his books like they were watered down fairy tales.
Also, horror movies simply don't affect me in any horrific or cringe-worthy way.

As an adult in reality, nothing scares me. But when you are a kid and teen, it does work on many. Just the music alone can affect people. The first 4 "Friday The 13th" when I was a kid/teen, made me squirm.
But as old as I am now, nothing scares me as far as violence in movies. I do occasionally wince now, but in the context of "how did they do that"?

There's also a scene in Ju On where a high school character looks through a gap in the newspapers that were being used to block the window and sees all the bluish-skinned dead that the spirit had killed, including her classmates.

Most of Serpent and the Rainbow, especially scenes in with the chair. I was somewhat 'altered' when I saw it.

Back in the 1950's, when I was only in kindergarten or first grade, I saw a preview on TV for The Last Days of Pompeii. People were running though the streets on fire while Mt. Vesuvius erupted. I didn't crap my pants, but that night I had a nightmare in Technicolor and woke up screaming. It took my mom about an hour to calm me down.

That scene in Silent Hill (2006) when Pyramid Head has ahold of that cultist gal. He skins her by just grabbing a fistful of flesh and ripping it off her.
When I first saw War of the Worlds (2005) I became very upset by the scene just before the bridge and ferry when Tom Cruise and his kids lose the van they were driving. They were driving through a town, the road becoming congested with people on foot, the van the sole operating vehicle on the road after the EMPs had killed most transportation. People start banging on the windows, wanting to be let in, wanting to have a ride. Eventually, things escalate to the point that people start to physically break into the car, trying to tear Cruise and Justin Chatwin from the van. This one dude starts tearing barehanded at a hole in the windshield, hands bleeding on the glass, trying to shove his way through, people swarming, shoving into the vehicle, just piling on top of each other. It was an abandonment of logic(there was nothing reasonable or rational about the way they kept flooding the car with their bodies, just blindly trying to enter it on top of each other), a full embrace of primal fear and desperation, and completely believable to me(and terrifying) to think of people getting to that place of pure survival instinct. I can't imagine having to defend myself and my family against that, the only thing putting a stop to the madness, the sharp report of a gun.

I don't know about shitting my pants but there is a scene in the TV movie "Roswell" (1994) which is obnoxious trash movie btw, but there is a scene at the end where they talk to the alien lying on the bed that survived the crash, and at one point it becomes very life-like which still creeps me out when I see it.
