The same way we determine any other fact. Moral realism is a non novel position. It treats assertions to moral facts the same way we treat cat facts, for example.

If someone tells me that masturbating is bad because it'll make me grow fur on my palms I'm going to arrange for a vigorous experiment.

That's not testing a moral claim, that's testing a physical claim???

Moral realism...treats assertions to moral facts...the same way it treats any other assertion to fact.

Yes, you've said that. I've agreed, for the sake of argument that moral facts exist. Now I'm just querying the methodology of finding out what those facts are.
So, it's quite simple to devise a test to find out if cats exist or if there's one in the room or if they can fly and so on. But what test can we devise to find out if causing harm is bad?
A question that's been asked and answered twice in as many posts.

You asked how we would arrange for an experiment directly after having two experiments described to you. Furiously masturbating to test the theory that masturbation is bad because it will make hair grow on palms, or pouring a pot of boiling water on an elderly persons lap to see if harm really is bad - or if that's just...like, my opinion, man.
