Yes. "Metal" is suggestive of the sound of a piece of metal being scraped or scratched. Think of the noise from banging pots and pans together, and scraping on them with a fork. Only think of it done on an industrial scale. The "extreme" is telling you how annoying and terrible it is.

I didn't sign anything. I grew up in a country that spent ungodly amounts of money building weapons possibly capable of destroying humanity. (I still say, if we don't use them, it will be the biggest waste of money in human history.) Besides, some of the leaders here are such fuckers that they didn't follow the Geneva Convention (e.g., Cheney, etc.), so you should expect that I might be willing to do something really vile and unspeakable. So watch out!
