Trump may not be the establishment, but he almost certainly is one of those pulling the strings.
I don't think you know what ultra rich really is. In fact most people that use the word establishment don't really understand it. I use it because it's something people can relate to, but it's an ideology that unites them, not just wealth. They despise the common people, and they believe we are all both on the left and right too stupid to understand so it's up to them to implement policy. Democrats that believe the establishment is Republicans, are the most disillusioned, but there are also the right that think the left has taken over. Both are right, but it's really just something they use to keep us fighting with each other so we don't wake up to the fact they are controlling both parties, and us.
People wouldn't understand the concentric circles of power which unite them, and as you go further up, it just gets stranger. We simply can't relate, because we are actually good people. If I even talked about the bad stuff at the top no one would believe it, so some things are better left unsaid.
