I'm not sure what a fair rate is. Either we pay what they ask, someone (or many someones) pay what they ask, the doctors have to work for free....or we don't get care. We're sort of bound in on all sides, don't you think? Insurance (where we all pay in and what they ask comes out) arose as a response to the increasing cost of healthcare...the point at which it became apparent that none of us could afford treatment. What's the alternative?
