Nothing to do with costs, but several years ago while on holiday in Florida my son got a nasty sunburn (big ugly blister on this shoulder) we went to the local doctor who sorted him out. No problems at all with the treatment but what made me laugh was that he said we should get him checked out by our doctor in London when we got home. I pointed out that we lived about 250 miles from London, but he insisted we should see our doctor in London. I assume that it was the only place in England he had heard of and couldn't imagine anyone living anywhere else!
