Personally I think some of the difference lies in the way in which the people are going to die. In the trolley example, if the 5 wretched mortals die, it will be because a trolley ran over them. If the 1 wretched mortal dies, it will also be by a trolley running over him. In the organs example, the 5 people will die of organ failure, because for whatever reason they are already unhealthy and have not been able to become healthy in time to live. But the 1 person, if he/she were to die, would be a healthy person, (assuming that whatever minor problem he/she's come to the hospital for is not going to kill him/her), who died because she was deliberately made unhealthy.
That, personally, is why I find the consequences of saving 5 people to be quite different in the two situations. So yep, inconsistent.
