Right, he's in there for observation because he appears insane. They simply would not commit someone to a mental hospital for mere dehydration/malnutrition so there must be other factors involved - not to mention how inconsistent his highly active physical behaviour is with that of other suffers of malnutrition and dehydration - While you could reasonably expect these people to harm themselves via lack of spatial awareness, wandering into traffic for instance, they simply could not muster a display that is tantamount to a rampage, running around, screaming, jumping, punching and kicking cars and the sidewalk and then masturbating - they would simply lack the energy to undertake these actions, let alone their being likely incapable of attaining an erection or even being disinterested entirely.
And let's not overlook the fact that someone in his position running a charity that took in 14 million last year who has ample access to food and water and literally has a team of people who would bring it to him there is a pretty damn high probability that, even if he was actually "malnourished and dehydrated" his mental state wasn't that good to begin with.
