The fear can be reality based, like fear of flying or cars. But it's the level of distress being incongruent to the perceived threat that's irrational.

My sister was scared of fluff. It was ridiculous. Out of nowhere you would just hear "Aaaaaaaaagh! FLUFF!" then see her sprint away.
I have a cousin with food phobias... He is in his mid-20's and only eats a very limited diet (mainly pancakes and chicken nuggets). His sisters used to chase him around with crusts of bread... Which is mean, but damn, it was funny to watch a 12 year old running in terror of bread.
