First world problems, doesn't literally mean 'first world problems'. If it did, then many people in the third world would have first world problems. First world problems is simply a phrase we use when commenting on complaints about minor things that are just slightly annoying or awkward. Having nude pictures of yourself stolen, copied and distributed across the internet, is neither merely annoying or awkward.

If you want to define first world problems like that, then they don't exist. All the problems that the third world has, are also in the first world. Starvation, disease, rape, murder, all happen in the first world too. Is the only difference here, the difference in occurrence of these things?
