Scenario A&B: Consent was not given. If she's asleep, even if she was apparently previously permissive, that does not mean consent
Scenario C: She's not of age to consent. I understand the circumstances but it's up to him to be careful. If she had a fake ID or something like that, then there are mitigating circumstances.
Scenarios D-F: I will agree that these should be punished very harshly
But, as rexbeccarox says, there are multiple different crimes in all these scenarios, not just rape
Absent further information, A lacks consent and is rape However I can envision relationships where open-ended consent is given because the female finds that sort of scenario arousing. As written, yeah it's rape.
