Just for the record, there is a procedure for gender reassignment and it's a hell of a lot more complicated and lengthy than "cut my dick/tits off!" "Okay." I don't know this from personal experience, but here's a grossly oversimplified version of what I understand has to happen before a given transperson is allowed to do the surgery:
  • A psychiatrist (likely two, especially in Europe and America, where two psychotherapists' approval is required for the genital surgery) must spend months evaluating said transperson to make sure that this is the right decision for them, whether they can handle it, whether they're likely to regret it, or whether they're likely to turn into Buffalo Bill (note: in the original novel, Billy-boy failed this stage, due to his obvious mental instability and the fact that, well, what we see of his reasons behind wanting to be a woman would likely be well outside what would make sense for actual trans experience, and this rejection seems to have led to his becoming a serial killer.) Some nations, like Thailand, skip this stage; most don't, and those that do still require the next step.
  • If the psychiatrist agrees that they were born into the wrong gender, they will be okayed for hormone replacement. This will, of course, include a long adjustment period to make sure things are still working properly. 
  • The RLT: The vast majority of jurisdictions that practice gender reassignment surgery require the patient to live as their preferred gender in as many ways as possible for a period of at least a year. To be fair, some minor surgeries are allowed in this period, like facial feminization surgery, but the big chop will have to wait at least a year. In addition, transwomen have to be on hormones for 18 months before they get breasts (for transmen, it's significantly shorter to get theirs removed). In addition, some psychiatrists actually require the RLT not only before surgery, but before they sign off on hormone replacement. 
  • After all this time, a minimum of two years after the initial psychiatric visits, then and only then, does the patient finally get their genitals to match what they wanted it to be all along, and it usually involves plastic surgery to remove their existing genitalia and remold it into a form they're more comfortable with.

I can remember we had two transwomen on the board a long time ago, but I don't see Violet or TaraJo's names pop up too often anymore. Some more may have still come on in their place, and if there's more steps than even this, or if I greatly misrepresented anything, a correction would be nice.

You'll notice that this is a lot different from just saying "I think I'm a girl, cut my dick off" to a doctor and expecting them to say "okay." You will also notice that this is different from someone just demanding amputation of a healthy limb.

ON TOPIC: It's worth reading about Christine Jorgensen, the very first transgender person I ever heard about. I think this was in the late 1950's

Christine Jorgensen (May 30, 1926 – May 3, 1989) was an American trans woman who was the first person to become widely known in the United States for having sex reassignment surgery in her 20s. Jorgensen grew up in the Bronx, New York City. Shortly after graduating from high school in 1945, she was drafted into the U.S. Army for World War II. After her service she attended several schools, worked, and around this time heard about sex reassignment surgery. She traveled to Europe and in Copenhagen, Denmark, obtained special permission to undergo a series of operations starting in 1951.[1]

In Australia, we have 'Carlotta', a high profile trans entertainer and TV personality.

Carlotta (born Richard Lawrence Byron; 2 September 1943)[1][2] is the stage name of Carol Spencer, a transgender Australian cabaret performer and television personality. She began her career as an original cast member of the long-running male revue Les Girls (pronounced lay-girls) cabaret show, which started in 1962 in Sydney's Kings Cross. Carlotta performed spot-numbers as a singer and comedian, and eventually became the show's compere and lead attraction.
