This is interesting. I, of course, have seen people rudely take up the whole aisle in a grocery store, but they always move when I say, "Excuse me." Since I have lived near both the east and the west coast, and in a few places in between, I do not think this is primarily a matter of location.
Maybe it is my godlike appearance that commands compliance. Either that, or you are not saying it right somehow. Frankly, if I had to say, "excuse me" more than twice, I would be moving their damn cart out of the way regardless of what they did. But I have never had to actually do that.
Just to be clear about a couple of things, although I am taller than most people, I am not exceptionally tall. And although I do lift weights, I am not extremely muscular. I am not exceptionally large, nor am I small. I do not have any visible tattoos, nor do I look particularly scary. When I let my hair grow and grew a full beard, I looked a lot like the depictions of Jesus one sees from northern European paintings. Most of the time (like now), I have no beard and short hair.
Regardless of the cause, I have never had to say "excuse me" more than twice to get people to move from blocking an aisle in a grocery store. A couple of times, it seemed like they did not hear me the first time, so I was a little louder the second time to make sure they heard me.
Also, due to my schedule, and my wife's schedule, I do most (almost all) of the grocery shopping, and always have, so it is not due to a lack of time in grocery stores.
So, is it that you are doing it wrong, or are people awed by my majestic appearance? Inquiring minds want to know!

If you are as you describe, I'm certain it is the latter. My picture has been posted all over this board, so you can see I am not menacing and perhaps I am too nice.

Do you think the typical depictions of Jesus in northern Europe make him a menacing figure?
By the way, to get the right idea, you need to look for blue-eyed versions, with only slight curls when my hair is long. When very short, no curls are apparent.
My wife says I am the cutest man in the world. So you can be sure that that is an unbiassed description of me.
I am really not menacing looking. And I have never been in a fight in my adult life. I have no wish to cause trouble when I go out and about, but I have limits to how much crap I am willing to tolerate from others. Like when they rudely block a grocery aisle with their carts.

As an aside, it was very interesting when I had long hair and a full beard. If I had my hair in a pony tail, no one paid too much attention to me. But when I had my hair down, people really looked at me quite a lot. I particularly remember this when I was in an art gallery looking at art, that people seemed to look at me about as much as they would look at one of the pieces of art. They were reasonably discreet about it, as they would avert their gaze if I looked directly at them.
If I want a lot of attention, all I need to do is grow my hair out and grow a full beard and I will get it. Unfortunately, I do not know what the strangers were thinking.

If you want to get mad, try talking on the phone with a customer service rep whose accent is so thick you can hardly understand. whether you live in the US or the UK, it's maddening to realize you're paying these people to outsource jobs.
If I were president or prime minister or whatever, I'd make a law, it 75% of your customers are here then 75% of your employees must be here.

Shop clerks who are doing something else when they're supposed to be attending to me. You're on the phone? Hang up. You're re-stocking something? Do it later. You're having a chin wag with a co-worker? It'll wait til I'm done.

There's something about the sight of any person over the age of five picking his/her nose that makes me borderline violent. I mean, damn, we all have done it, but I don't think it's something that needs to be shared.
