I've never had to report anything to HR, just saying. Probably because everyone around me are decent human beings who wouldn't go out of their way to offend.

"Wouldn't go out of their way to offend" - can you expand upon that? As far as I am aware, none of the situations I am talking of went out of their way to offend - and as I have explicitly said multiple times now, anyone who does should be terminated on the spot, fired, without further question.
Even if they didn't go out of their way to offend, one should always think before speaking. Consider the location and the audience, then speak accordingly.
The medical field is highly advanced in most respects, including areas of ethics. Just as someone can be punished for neglect, even if it was unintentional, the same applies to one who speaks in a manner that may offend.
