Without reading all three pages here, I would add:
Proselytizers/any "believer" who comes to an atheist forum for the sole purpose of trying to save my soul (or pretending same) and/or comes to an atheist forum to tell its members they are wrong and going to hell for their personal beliefs (or lack thereof, however you want to phrase it).  

I  have never once in my entire life sought out a forum for the express purpose of trying to shove my beliefs (religious or otherwise) on anyone or tell them theirs are wrong.  At its BEST that is just plain rude.

There are two people here I have considered putting on ignore. One is a racist anger ball and the other - I don't know how to classify.

Nearly seven years on TTA and most of my ignore list consisted of people who ended up banned anyway and perpetual whiners.

I don't think there's one list. We all have people we simply don't want to deal with...online and IRL.

Don't know about others, but for me personally, 2a is more like "evidence is demanded and not given, when said believer keeps claiming there is evidence"

If you're going to say there's evidence, you better make at least some effort to provide it. Otherwise, I'll think you're just full of shit.
