People are forgetful in nature, and we forget many things. Sometimes we forget if we have paid the bills this month, if we have had dinner, and if we had a kid with us this time. The chance is higher if it's not a habit, so say the dad picks up the kid from school every day, but just for today you are supposed to pick it up. But it's quite easy to forget that unless you set a reminder of some sort.
I don't blame the parents, but I do hope the general awareness about this increases a bit. When going out with a baby, is it really necessary to close all of the windows entirely? And is it too unthinkable to put a tracker or monitor on the kid so that it can warn them if they go a certain distance away from it? Also I support the cellphone on backseat idea. It's easier to locate your cellphone than your sleeping baby
