It has two options. If you believe in some sort of God (doesn't matter if you are a Christian, a Deist, a Wiccan, etc) then answer "Yes". If you don't believe in God (and that includes people who don't think the question is relevant), answer "No".
I just want to keep things very simple; I don't want to cause a debate over terms; if you think you believe in God, answer in the positive. If you don't think you believe in God, answer in the negative. If you are unsure what you believe, don't answer at all.

I used to, and my family still does, though they are moderate christians. They don't believe in Hell and they don't hate other religions or atheism.
My dad believes in an afterlife though, and he said more than once that the most important thing is to pray to Jesus.
My mom is uncertain about the afterlife and doesn't see other religions as inferior, but she once said that she will never change her religion or her domination. I asked her why, she replied me "Because I feel so".
She told me, that we may never know if a God exists or not, but praying gives you positive energy. WTF?
So she's clayming that you need to imagine that an invisibile being exists and protects you in order to be happy, rather than seeing the world for what really and surely is?
I really love my mom, but this statement is just silly.
