Self-awareness for the third time is controlled by the brain stem, maybe you would understand it if I called it basic awareness. What you are asking is how can one have higher functions of self-awareness without any cortical influence. Nobody ever said Nicholas has higher functions and considering his missing neocortex that would be an impossibility.
