One of my brothers was involved in the development of the P-125 (cancelled) for the RAF. He got a commendation for pointing out that ‘Fuel Level’ had been left off the preliminary checklists.

A boyhood friend of my dad is also a pilot - and for years owned a Cessa 310. (One of my favorite light twins). Dad had flown up to see him - and they hopped in the 310 for a quick run around the field. Less than a minute after getting airborne (but thankfully above V2) one engine cut out. Dad was sitting left seat (he has far more flying hours) He was into procedure and had just feathered the out engine - when the other engine went.
This is in heavily wooded NE Wisconsin. The only place to go was were they had taken off - or a treetop crash. They barely made it back to the field.
Somebody had siphoned the tank.
And they missed it.
Dad says - sometimes you survive stupid.
But only sometimes.
