Yep, the first doctor originally looks down upon humans as a petty lesser race, his first human companions he kidnaps against their will by refusing to let them leave the TARDIS all the while laughing at them!

He has essentially left his own race behind, he dislikes timelords and has more than likely betrayed them. Hence why he later adopts humanity as the race with whom he identifies most ...

In my mind, they've already solved that dilemma.

River Song gave up all her regenerations to save the Doctors life. She'd only regenerated twice, so that means she had 10 regenerations left. At best, one could argue that the regeneration energy from her 2nd regeneration was enough to heal the Doctor, so he has 10 more. At worst, he at least has a few more to use.

I'd imagine that scenario wouldn't be so hard, compared with a scenario where a Time Lord's partner changed gender. I mean, regeneration alone must be hell for relationships, given the new personality and appearance that usually comes with it, but what if you're a Time Lord who only likes Time Ladies, and your Time Lady partner regenerated into a Time Lord (or vice versa). That's got to be awkward.
