Just got out of a staff conference earlier. Lost a patient on Monday and now the whole investigation is under way to determine if anyone screwed up.

*sigh*

Only the second patient I've lost on the table.

First was a few years ago before I even started surgery.  This one was nearly an hour into a routine surgery.

All attempts to revive failed.

Damn, that sucks. Sad  I'd make a "blame the intern" joke, but somehow I don't think it'd go over well. Undecided
